# 6.2.5 – Documentation Requirements

> Documentation requirements for medical intervention by law enforcement officers.

Any medical intervention by an officer must be documented in the incident report or medical notes, including:

* Aid provided
* Reason intervention was required
* SAMS status (on duty/not on duty)
* Any directives given by EMS
