# [2.2] - Department Rank Structure and Duties

> Medical Services rank structure and duties from EMR through Medical Director.

### 1. Emergency Medical Responder (EMR)

**Role Summary:**\
Entry-level responder. Focuses on stabilizing patients until higher-level care arrives.\
**Responsibilities:**

* Provide CPR, bleeding control, and basic airway management
* Assist EMTs and Paramedics during field calls
* Observe and learn EMS protocols
* Cannot operate solo — must be with a certified EMT or higher

### 2. EMT I – Basic Emergency Medical Technician (EMT-B)

**Role Summary:**\
First level of certified EMS responder with field autonomy.\
**Responsibilities:**

* Assess patients, apply splints, perform CPR, monitor vitals
* Transport patients to medical facilities
* Operate ambulance and assist AEMTs and Paramedics
* Complete call reports and patient handoff paperwork

### 3. EMT II – Intermediate Emergency Medical Technician (EMT-I)

**Recommended Name: "Intermediate EMT"** — this is a real-world bridge level recognized in some EMS systems.\
**Role Summary:**\
Expanded care technician with skills beyond Basic, but not as advanced as AEMT.\
**Responsibilities:**

* Perform EMT-B functions with additional interventions (e.g., glucometer, airway adjuncts)
* Assist with IV setup and medication prep (under supervision)
* Serve as acting lead when no higher-ranking EMT is present
* Begin field leadership development

### 4. EMT III – Advanced Emergency Medical Technician (AEMT)

**Role Summary:**\
High-skill field technician with critical intervention authority.\
**Responsibilities:**

* Initiate IVs, administer limited medications, manage cardiac events
* Lead scenes involving trauma, overdose, or critical illness
* Collaborate with Paramedics and hospital ER staff
* Document patient status thoroughly and ensure continuity of care

### 5. Paramedic (Supervisor-Level)

**Role Summary:**\
Highest field-certified EMS responder. Supervises on-scene medical operations.\
**Responsibilities:**

* Perform advanced airway, cardiac, and pharmacological interventions
* Supervise EMRs and EMTs on scene
* Lead high-risk or multi-casualty incidents
* Handle field training and skill verification
* Coordinate with law enforcement and hospital staff

### 6. Lead Training EMS (Lieutenant)

**Role Summary:**\
In charge of training, mentorship, and certifications.\
**Responsibilities:**

* Oversee FTO program and training progression
* Assign field evaluations and provide feedback
* Maintain skill logs and exam records
* Report to Head of EMS
* Ensure all lower ranks meet department standards

### 7. Head of EMS (Captain)

**Role Summary:**\
Command-level supervisor of all EMS field operations and personnel.\
**Responsibilities:**

* Manage daily EMS structure, scheduling, and personnel issues
* Approve field protocols and training direction
* Coordinate with Medical Director on standards
* Represent EMS in command-level meetings

### 8. Director of Pharmaceuticals

**Role Summary:**\
Authority over medical inventory and controlled substances.\
**Responsibilities:**

* Oversee medication use, storage, and audits
* Maintain narcotics logs and supply chain integrity
* Train Paramedics and AEMTs on med administration protocols
* Report directly to Medical Command Staff

### 9. Assistant Medical Director

**Role Summary:**\
Second-in-command of medical operations and protocol development.\
**Responsibilities:**

* Enforce quality control and evaluation policies
* Approve EMS certifications and rank promotions
* Handle disciplinary reviews and SOP changes
* Act as Medical Director when absent

### 10. Medical Director

**Role Summary:**\
Top of the medical command structure. Sets all treatment standards and oversees legal/ethical conduct.\
**Responsibilities:**

* Final authority on all EMS protocols, certifications, and practices
* Oversee staff conduct, hiring, and terminations
* Serve as government and hospital liaison
* Guide policy, field care innovations, and department expansion
