# [6.8] - Reports

> Medical incident reporting requirements and report template.

Reports are to be made regarding any and all PD related medical incidents, including but not limited to: Treating 10-95s, treating downed officers, responding to a completed pursuit, 51-50’s (Psych Hold) regarding Officers. Reports will also be made about civilians, including but not limited to: Gunshot wounds, Surgeries, ICU stays (put in ICU Reports tab). Reports are to be done for SERIOUS TREATMENTS ONLY. Use your best judgment regarding proper use of reports. See the below template

Command staff will review and verify reports. If they are done inaccurately or not at all, disciplinary action may be taken.
